Understanding Incident-To Billing and E/M Requirements

The following Understanding Incident-To Billing and E/M Requirements is designed to educate healthcare providers (HCP) on the key principles and compliance requirements of Medicare’s incident-to billing and Evaluation & Management (E/M) coding. This course helps HCPs accurately document, code, and bill services while maintaining full regulatory compliance.

• Supervision, scope, and documentation standards

• Proper use of E/M codes 99212–99215

• Selecting E/M levels using time or medical decision making (MDM)

• Medical necessity and compliance best practices
